Thank you for considering the Winston-Salem Street School as a part of your education. The following forms need to be submitted to the office in person, by mail (415 West Clemmonsille Road), e-mail (wsalemstreet@bellsouth.net) or by fax (336-788-3336).
- Student Application
- Autobiography-1 page paper stating personal goals you have for yourself and for your future
- Transcript from previous school
- Immunization form-if not included with transcript
Once ALL of these items are received and space is available, the principal will contact the student to schedule an interview.
When the student comes in for an interview, a parent's/gaurdian's income verification (W-2 form from previous year, previous year income tax return, or three most recent paycheck stubs) will be required in order to determine tuition.
